Items You MUST Bring:
1)  Completed Emergency Care Form – you will NOT be allowed to register without it. (enclosed)
2)  Completed Internet Use Agreement – printed on the reverse of your Emergency Care Form– signed by   student AND parent.
3)  Authorization for Education Records Disclosure (email to parents) OPTIONAL – printed on reverse of
     Emergency Care Form
4)  2012-13 Activities Contract – signed in BOTH places by student and parent.
5)  Completed fee sheet and money to pay for registration fees.
6)  Fees, fines or items left unpaid at the close of the 2011-12 school year.  (i.e. $5 fee for student ID, lost
     locks, book damage, etc.)
7)  PATIENCE.  We will do our best to serve each of you promptly.  Thank you! 

Class schedules:  Based on selections made by students at pre-registration in the spring, student schedules are available to view through the student portal in Lumen/Websis.  Some changes have been made to the Master Schedule which may necessitate a change when you come to register.  A finalized copy of each student’s schedule will be printed, and be available for pick-up at the High School Office window beginning at 7:30 a.m. on the first day of school, August 29.

Fees:  A fee sheet has been enclosed. For your records and ours, the preferred method of payment is by personal check.  The cleanest accounting trail is to provide separate checks, one for each registration form or student. We do not, at this time, have the capability to accept Debit/Credit card payment.

Students planning to participate in activities (sports, music, clubs, etc.) MUST purchase a Student Activity Card.  If participating in sports, or taking a PE class, you must pay the towel fee.  Jr. High Students will NOT be charged for Industrial Arts or Art Classes. There are no class dues for Jr. High Students.  All other club dues are listed on the form.  YEARBOOKS, which will reflect the 2012-13 school year (with emphasis on High School) are optional, but can be paid for at registration.  Yearbooks are $35, ONLY at registration.  After registration, the price will be $40.  *Please do not include Pay-to-Participate fees or lunch money with registration fees.  Those items need to be paid separately.

For your convenience, lunch money will be accepted at registration.  It is very helpful to the food services department to apply money to lunch accounts prior to the first day of school.  Should you choose to wait until the first day of school, please take those funds to the lunch room as you arrive in the morning, rather than waiting until lunch time to pay.  Thank you.

The fee sheet also lists the costs for season passes.  *Family Passes will be $175.  Purchase of a family pass covers the cost of activity cards (the $40 activities admission and participation portion, not the $10 surcharge) for students in your family.  Therefore, as an example, if you will be purchasing two activity cards at registration ($40 X 2 = $80), the family pass will be $95.  This applies to the cost of a Family Pass ONLY, and students will still need to pay the $10 surcharge.  Season Passes will be available for purchase at registration, and daily at the High School Office.

Immunizations: Regulations have changed for students entering 7th grade. Please check with your physician to make sure your child is up to date on immunizations. Students entering 7th grade are now required to have a TDaP booster and the Meningitis vaccination.

Locks and Lockers:  Lockers have been randomly assigned to students through the student management system.  Locker numbers and lock combinations will be printed on student schedules.

School Supplies:  For the first day of school, Jr/Sr High students will only need paper, pens and pencils.  As students rotate through their schedule, they will be informed of any special supplies needed, if any.

Sports Forms:  As a reminder, all students planning to participate in sports must have a current physical on file.  ALL 7th, 9th and 11th grade students must have NEW physicals (May, 2012 or after).  Physical forms are available at the school website at www.sd282.org  and at the school offices, as are Pay to Participate forms.

School Calendar:  A 2012-13 school calendar has been printed on the reverse of the fee sheet. (The fee sheet is simply a worksheet for your convenience.  A printed copy of the receipt for fees will be given to students at registration, so the worksheet/calendar copy can be returned to you.)  Please remember, the school day begins promptly at 8:00 a.m.  The school day for secondary students ends at 3:20 p.m.

The school district website at www.sd282.org  is your best resource for back to school information, events and schedules.  Please call the school office at 285-1162 if you have any further questions or concerns.

*Remember, school begins Wednesday morning, August 29 at 8:00 a.m.  Finalized Schedules will be available for pick up at 7:30 a.m. We hope the remaining days of your summer are fun and safe, and we’ll see you soon!
